    Cancer care includes multiple treatment modalities delivered in complex multi-team organizational systems. This complexity earned the Institute of Medicine label of "system in crisis." Little research has explored critical areas of care coordination between patients and physicians during inpatient-to-ambulatory care transitions or during transitions across phases of the cancer care continuum. The same is true of care coordination between health providers.  This multi-perspective discussion will explore how to measure cancer care coordination.  We will be referring to the attached case study during the discussion.
