Cancer care includes multiple treatment modalities delivered in complex multi-team organizational systems. This complexity earned the Institute of Medicine label of "system in crisis." Little research has explored critical areas of care coordination between patients and physicians during inpatient-to-ambulatory care transitions or during transitions across phases of the cancer care continuum. The same is true of care coordination between health providers. This multi-perspective discussion will explore how to measure cancer care coordination. We will be referring to the attached case study during the discussion.
The HCT Cyber Discussion series, sponsored by the Healthcare Delivery Research Program, explores how healthcare teams utilize health information technology to improve cancer care delivery. Please visit our website for more information or email the HCT planning team with inquiries.
