My Own Health Report Learning Community

The R2R MOHR Group is a community of practice based upon the experiences of the My Own Health Report (MOHR) project team.  The MOHR project tested the implementation of an evidence-based patient-reported health assessment and feedback system for use in primary care. This is significant since the Affordable Care Act mandates an annual wellness visit including a health risk assessment for all Medicare patients. The project, a collaborative effort of six research teams, and 18 primary care practices  was funded by the Office of Behavioral and Social Science Research( OBSSR), the National Cancer Institute (NCI) and the Agency for Health Research and Quality (AHRQ) under the administration of the NCI Implementation Science team. The purpose of the R2R MOHR group is to continue the collaborative innovation begun in the original project.  Building upon lessons learned we intend to further develop the MOHR process and expand upon it. We wish to stimulate creative thinking grounded in practice experience as well as science so as to move the lessons learned from MOHR into practice in various areas of healthcare delivery research with a special focus on cancer control.
